The article explores the challenges of developing public health guidelines when the underlying evidence is of low quality. It marked the final publication of a highly successful collaboration with an outstanding team of experts in meningococcal disease. We shared our experience of producing consistent recommendations for the management of contacts of invasive meningococcal disease in the European context, drawing on a review of the existing literature, the formulation of key research questions, and the application of the GRADE methodology. Despite the limitations of the evidence, we issued both strong and weak recommendations, highlighting the need to consider factors beyond evidence quality, including patient burden, values, and resource implications. This approach may be useful in other areas of public health where robust evidence is often lacking.
